Problem for validate the form

Hi sir,
I`m facing problem at the end of validating form
some fields are not being checked.. (radio button, select box, and check box)
date field not being checked when i insert field hint function (means when focus the text field a massage showing some detail about the text box)
i mailed you which i`m having trouble.
