A better patching program or an admin simply who can oversee these is what organizations can always come up with. But, unfortunately most of the admins just scrap off the patching job from their to-do lists.  

Another instance is the patching of software’s like Java or PHP. If a firm finds a random error after patching these applications, then a sudden notice pops up saying that further updation is forbidden. Instead of finding the compatibility problem, admins’ like to stop the update, thinking that they are safe without patching, which is always is a better lead to hacking. And still this process is continued for years.
