People Affected: 800,000

Brief: In March this year, private data including social security numbers, names, addresses, driving license numbers, addresses, health insurance information etc of around 800,000 adults and children from four computer storage devices was lost by IBM and Iron Mountain during transit between Boulder and Sacramento. It has been said that the incident occurred when an unsecured container fell down that FedEx was transporting.

13. Utah Department of Technology Services
Where is it: Salt Lake City, Utah
When did it happen: April
People Affected: 780,000

Brief: A hacker from Eastern Europe hacked the Utah Department of Technology Services server and procured the critical information of nearly 780,000 patients. Due to this grievous incident, the CIO of Utah DTS had to resign the following month. The incident has led to a fear of identity theft among those affected.
