Louise Fletcher received the Oscar for Best Actress for her performance in this film. Her role as autocratic nurse rules over the psychiatric ward of a mental hospital got her a title of fifth worst movie villain by the American Film Institute. She controls the patients by increasing the pills and abusing their egos. This even leads to the death of one young patient.
